Kate Hazzard, PA-C
Kate was born and raised in Portland, Oregon. She graduated from Whitman College in Walla Walla, Washington with a Bachelor of Arts in psychology and a minor in chemistry. Prior to PA school, Kate worked at OPA as a medical and research assistant to Dr. Crutcher, and then continued her education at the University of Washington’s Physician Assistant Program here in Seattle.
Kate is very excited to be a part of Dr. Crutcher’s team at OPA, and looks forward to seeing patients from the past as well as new ones. In her free time, she enjoys biking, hiking, playing tennis, and spending time with her husband, Jesse, and golden retriever, Norman.
